You can order online and pay using PayPal or a credit card, or you can print the order form fill it out and mail it in with a check.   Click HERE to print the order form.
To offset the cost of using PayPal, there will be a $2.00 charge for each online order.  This line item is MANDATORY for each order.  It will also make a record of your name and your club name.
